Fire Prevention Supply Request
By checking the Agreement to Terms of Use box I, on behalf of my organization, am requesting fire prevention supplies from the Office of the State Fire Marshal for the State of Kansas (OSFM) and upon receipt of said materials I agree to abide by the terms and conditions for the use of the materials as set forth in the Fire Prevention Supplies section of the OSFM website. Specifically, I agree to dedicate the required amount of time for fire prevention activities. EDITH DVD: I agree that if I take possession of the Edith DVD and said DVD is lost, stolen, damaged or otherwise rendered unusable while in my possession, I will reimburse the OSFM the monetary replacement cost of the DVD.
